Application for Membership
I, ____________________________________ of ________________________________ have read and understand the rules of Yueh Fei Kung Fu as set out, and do hereby apply for membership. I agree that if accepted for admission to training in Yueh Fei Kung Fu under the rules of the Principal Instructor and authorised instructors, I will abide by those rules. I understand that Rule 17 (xvii) will apply only on discretion of the Principal Instructor.
I further agree that should I incur any injury in any form as a result of or during the course of my training within the environment in which or through which Yueh Fei Kung Fu exercises are conducted under the Principal Instructor and authorised instructors, the aforementioned instructors and proprietor / agency of the environment in which or through which training is or has been conducted, shall not be held liable.
I therefore understand that if accepted for membership to Yueh Fei Kung Fu, I train at my own risk.
